Child Membership Form

Children 12 and under must have parental/guardianship signature before a library card is issued to a child. 

In keeping with the Ontario Library Association’s Children’s Rights in the Public Library: Guidelines for Service, there are no restrictions on the material borrowed by children. While the library staff can advise children on reading interests, the material selected by the child is the responsibility of the parent or guardian.

I hereby agree to obey all the policies and procedures of the Otonabee – South Monaghan Public Library Board, to pay promptly all charges against me for the damage or loss of materials and to give immediate notice of any change of address. I understand that my borrowing  Privelleges can be revoked at anytime under the descretion of the CEO and or library staff if the above rules and regulations have not been met.

I am signing on behalf of my child who is under the age of twelve and give permission to Otonabee – South Monaghan Library to allow my child to borrow materials from the Otonabee – South Monaghan Public Library.
